Position Description: Registered Nurse (RN)
Reports to: Executive Director
Created: August 5, 2012

Job Summary:  The RN is responsible for overseeing medically related needs of the individual served.  This includes, but is not limited to, making doctor’s appointments (primary care, psychiatric, dental, and specialty), attending doctors’ appointments, ordering medications from the pharmacy, checking for accuracy, and distributing to staff.  

  1. Essential Job Functions:



  • Serve as the liaison between physicians, the pharmacy, and the agency

  • Supervise LPN’s as assigned

  • Train LPN’s as needed

  • Maintain a current knowledge of each individual’s medical condition

  • Provide or ensure provision of nursing care in accordance with physicians’ orders and recognized nursing standards

  • Administer or ensure administration of medications and treatment as prescribed by physicians

  • Conduct quarterly nursing physical assessments as defined by Title XIX regulations

  • Participate in Interdisciplinary Team  meetings

  • Maintain accurate and complete records of nursing observations and care

  • Train staff to recognize, interpret, and report clients’ conditions

  • Implement and monitor preventive health care services for individuals and instruct individuals in good health habits

  • Assist physicians in diagnostic and therapeutic measures

  • Serve on committees as required

  • Monitor and coordinate the services of physicians and medical providers to expedite medical services

  • Audit medication administration to ensure the absence of medical errors or documentation error

  • Create and monitor a medication count sheet anytime a controlled substance is ordered

  • Conduct training in homes and within other programs on interactions of various medications and their effectiveness on diagnosed illness

  • Monitor OSHA Compliance and Infection Control as well as monthly audits to include patterns of reoccurring injury or illnesses of staff or clients

  • Review client medical records not limited to doctor’s appointments, lab work, effectiveness of medical treatment and medications

  • Regular and punctual attendance 

  • Keep staff well informed regarding health related problems of the persons served; discussing reasons for treatments, medications, etc. with the appropriate parties

  • Provide in-service training to staff on medically-related issues

  • Ensure that all policies, procedures, regulations, and guidelines are followed per state and agency requirements

  • Visit each home monthly to review medical documentation

  • Complete monthly medical note on each individual by the 10th of each month

  • Complete monthly medication variance forms for state nurse

  • Process cycle fill medications monthly

  • Order medications and check for accuracy

  • Meet weekly with individual program staff to update on client medical issues

  • Send updated contact information and client list to pharmacy, physicians, and other staff as needed quarterly 

  • Administer staff Flu shots/TB screenings

  • Review medical charts

  • On-call availability for medical concerns



  1. Physical Demands:

  • Occasional travel

  • Constantly moves about to coordinate work

  • Regularly assist consumers physically by lifting and positioning them as appropriate

  • Occasionally moves and positions objects weighing up to 50 pounds

  • Continuously exposed to viruses and infectious conditions


  1. Special Demands:

  • Ability to meet the public in a manner that reflects creditably upon the organization and must have the ability to deal with consumers and staff in a manner that is conducive to harmony and their best efforts

  • Must be adaptable and have the ability to make decisions

  • May be required to attend seminar or job-related training courses

  • Must have understanding, patience and tact in dealing with consumers, their families or advocates and other agencies involved in providing supports for consumers

  • Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities

  • Must be able to prioritize work tasks

  • Must be able to work without close personal supervision

  1. Education
Graduation from an accredited school of nursing.  Must possess and maintain current registration.

  1. Job Knowledge
Must have knowledge of and technical skills of medical nursing procedures, first aid, and basic principles of nursing. Must know or learn about mental retardation, seizure disorders, autism and other associated diagnoses pertaining to the field of intellectual disabilities and geriatrics. 

  1. Training and Experience
Minimum of one-year clinically relevant experience required.  Experience working with elderly, or intellectually disabled preferred. Working knowledge of Windows 7, Office 2010 applications. Aptitude to learn other software programs as required for this position.


  1. Employment Variable
Some evenings and weekend work may be required to meet the needs of the individuals. Will be constant contact with staff, individuals served, their families, and other support agencies 


  1. Must have a good driving record which meets the requirements of Evergreen and proof of liability coverage
