Job Summary

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

Responsibilities

Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans

Conducting quarterly care planning conferences for each participant enrolled

Provide nursing care based upon physician's orders and daily assessment of each participant's health status

Participating in staff education activities and providing consultation facility personnel

Ensuring that nursing services are provided to the participant as specified in the nursing care plan

Administer medications and monitor patients and manage coordination of care

Document any changes in patient conditions and report on changes

Maintain accurate records of patient care and discuss observations with the supervisor

Other duties as described in job description

Qualifications

Currently licensed Registered Nurse (RN) in state of employment

Previous experience as a Registered Nurse is preferred

Excellent interpersonal and client care skills

Valid driver’s license

First aid/CPR certification required

Compensation: $75,000.00 - $82,000.00 per year

STARS Adult Medical Day Care Center is owned and managed privately serving the Camden Community with adult day services for seniors as well as older adults. Our goal is to help members improve their daily function, increase activity and promote better health. In this way, we hope to delay the onset of chronic illness which may arise from limited mobility or isolation.

A lot of seniors and disabled clients find that socializing with peers their age is helping them improve their disposition. Apart from the social and emotional benefits, STARS Adult Medical Day Care Center also has health care services on site. We have a nurse, therapist and a medical team that visits us periodically to check on the health of our members.
