We’re seeking a dedicated Clinical Manager to oversee all patient care operations, guide our clinical staff, and ensure that every patient receives the highest standard of care — right in the comfort of their own home.

Oversee patient care services and clinical personnel
Manage assignments, referrals, and care coordination
Ensure patient needs are continually assessed and plans of care are up to date
Communicate closely with physicians, caregivers, and interdisciplinary team members
Supervise professional and paraprofessional staff
Provide education and counseling to patients and families
Participate in Quality Assessment and Performance Improvement (QAPI) programs
Ensure documentation accuracy and compliance with regulations
Support emergency preparedness and staff training initiatives
Licensed Registered Nurse (RN) or licensed professional in a related clinical field
Graduate of an accredited program in applicable discipline
3+ years of home health experience (preferred)
2+ years of management experience (preferred)
Strong leadership and organizational skills
Current CPR certification
Active license in Pennsylvania, in good standing
Reliable transportation and valid driver’s license
Proficient with electronic health records (EHR) and computer systems
Excellent communication and clinical judgment
Ability to manage multiple priorities and deadlines
Passion for mentoring staff and improving patient outcomes
Commitment to maintaining compliance and quality care standards
Frequent travel within the Scranton and surrounding NEPA area
May work in patient homes, schools, or occupational settings
Requires standing, walking, and lifting up to 50 lbs.
Must meet applicable health and safety requirements
