About the Role

We are seeking a compassionate, dedicated Registered Nurse to provide high-quality in-home care to our patients. As a Home Health Nurse, you’ll deliver individualized care, support patient independence, and build meaningful relationships with those you serve.

Responsibilities

• 
  

  Conduct in-home assessments and develop patient-centered care plans

  
  


• 
  

  Provide skilled nursing care (medication management, wound care, monitoring chronic conditions, etc.)

  
  


• 
  

  Educate patients and families on disease management and safety

  
  


• 
  

  Document care accurately and timely according to agency standards

  
  


• 
  

  Collaborate with an interdisciplinary team to coordinate care

  
  

Qualifications

• 
  

  Active RN license in Kansas

  
  


• 
  

  Home health experience preferred but not required (training available)

  
  


• 
  

  Strong communication and clinical assessment skills

  
  


• 
  

  Reliable transportation and valid driver’s license

  
  


•
