As a Home Health Registered Nurse, here's a big-picture view of what you'll do:



  • Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans

  • Perform comprehensive assessments and ongoing evaluations of patients with acute illnesses, chronic conditions, injuries, and post-surgical needs

  • Develop, implement, and update patient care plans to promote optimal health outcomes

  • Administer medications, treatments, wound care, and other skilled nursing interventions

  • Monitor patient progress, identify changes in condition, and communicate findings to physicians and interdisciplinary team members

  • Educate patients and caregivers regarding disease management, medications, treatments, nutrition, and safety practices

  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals

  • Supervise and collaborate with Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other support staff as appropriate

  • Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements

  • Participate in case conferences, quality improvement activities, and discharge planning to ensure continuity of care


A few must-haves for Home Health Registered Nurses:



  • Graduate of an accredited nursing program

  • Active Registered Nurse (RN) license in the State of New Hampshire

  • Minimum of one (1) year of nursing experience; home health, hospice, rehabilitation, or medical-surgical experience preferred

  • Current CPR certification

  • Valid driver's license, reliable transportation

  • Knowledge of state and federal home health regulations and standards of practice

  • Strong clinical assessment, critical thinking, and care coordination skills

  • Excellent communication, documentation, and interpersonal skills

  • Compassionate approach to patient-centered care and family education


Preferred Qualifications:



  • Previous home health experience

  • Experience with electronic medical records (EMR)

  • BSN preferred but not required
