To Stand Out

Email Elena at [email protected] with your resume and a brief overview of your RN experience, home health and OASIS background, and what interests you about growing your career through a Floating Case Manager position.

Why Learn One Territory When You Could Learn an Entire Organization?

Most RN Case Managers become experts in one geographic area.

You'll have the opportunity to become an expert in much more.

As a Floating RN Case Manager, you'll rotate throughout different portions of our 21-county service area, stepping into territories where additional clinical coverage is needed.

But this isn't about driving to a completely different corner of the metro every morning.

Your assignments will typically last approximately three months, giving you the opportunity to actually become part of each territory you support.

Learn the patients.

Learn the team.

Learn the area.

Manage the caseload.

Then eventually take everything you learned and bring it somewhere new.

The result? A home health nurse with an incredibly broad range of experience.

See More. Learn More. Become More Valuable.

Every territory teaches you something.

Different communities bring different patient populations.

Different caseloads expose you to different clinical challenges.

Different teams introduce you to different perspectives and approaches.

And every rotation gives you another opportunity to strengthen your ability to walk into a situation, understand what's happening, and confidently take ownership.

Over time, you'll gain exposure that can be difficult to develop when you spend years working within one permanent territory.

For the nurse who wants to keep learning, this role gives you room to grow.

How Your Territory Rotations Work

You'll rotate throughout our service area based on patient census, staffing, and clinical coverage needs.

A typical territory assignment may last around three months.

During that rotation, you'll primarily work within that geographic area and manage an assigned patient caseload.

That gives you time to:

  • Learn the territory and develop an efficient routine
  • Become familiar with your patients and caregivers
  • Build relationships with the clinical team
  • Understand the needs of that patient population
  • Provide continuity throughout episodes of care
  • Become a dependable resource within the territory

When organizational needs change, you'll transition to another area and begin building that same familiarity there.

It's the variety of floating with enough consistency to actually become effective.

What You'll Gain Along the Way

As you rotate throughout our organization, you'll have the opportunity to:

  • Work alongside multiple clinical teams
  • Experience different patient populations and communities
  • Strengthen your OASIS and comprehensive assessment skills
  • Manage a variety of home health caseloads
  • Build relationships throughout the organization
  • Develop stronger adaptability and problem-solving abilities
  • Expand your understanding of home health operations
  • Become comfortable stepping into new situations quickly
  • Build a reputation as someone teams can rely on

For an RN who wants to continue developing professionally, that's valuable experience.

A Few Perks We're Especially Excited to Offer

  • $95,000–$130,000 annual compensation plus incentive visit opportunities
  • $6,000 in first-year bonuses
  • Full-Time employment
  • Structured territory rotations typically lasting approximately three months
  • Free access to our MedBridge Education Hub for continuing education and professional development
  • Licensure reimbursement when it's time to renew
  • Annual scrub reimbursement
  • Mileage reimbursement between patient visits
  • Cell phone reimbursement for eligible clinicians
  • Comprehensive medical, dental, vision, PTO, paid holidays, and 401(k)
  • Independence in the field with experienced clinical support behind you

What You'll Do

Regardless of which territory you're supporting, you're still the RN Case Manager responsible for helping lead your patients' care.

You'll:

  • Manage an assigned home health caseload within your current territory
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing visits
  • Perform wound care, medication management, and other skilled nursing services
  • Educate patients and caregivers
  • Monitor progress throughout the episode of care
  • Identify changes in condition and intervene appropriately
  • Coordinate care with physicians and other healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help reduce avoidable hospitalizations and support patients in remaining safely at home
  • Maintain timely and accurate documentation in Homecare Homebase (HCHB)

The Best Float Nurses Become the Nurses Everyone Knows.

Spend enough time working throughout an organization and something starts to happen.

You know more people.

More people know you.

You've worked with different managers.

You've supported different teams.

You've handled different patient populations.

You've seen how different territories operate.

And when something challenging comes up, there's a good chance you've encountered something similar before.

That experience compounds.

And it can make you an incredibly valuable clinician.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Strong comprehensive assessment and clinical decision-making skills
  • Home health and OASIS experience preferred
  • Confidence independently managing a patient caseload
  • Strong organization and time-management abilities
  • Excellent patient, caregiver, physician, and team communication
  • Ability to adapt quickly to new teams and territories
  • A collaborative, flexible approach to patient care
  • Reliable transportation
  • Willingness to travel throughout our 21-county service area as territory assignments change
