You are an RN who

• A licensed RN (or eligible for licensure in Wisconsin)


• Passionate about building meaningful connections with patients


• Comfortable working independently while supported by a collaborative team


• Skilled in both written and verbal communication
  
  

Key Responsibilities

• Serve as Case Manager, coordinating comprehensive patient care


• Conduct and document thorough assessments; develop individualized treatment plans


• Collaborate with patients, families, and clinical staff to optimize outcomes


• Maintain clear and concise documentation, including discharge summaries


• Educate patients and caregivers on safe discharge planning in coordination with the clinical team
