Schedule

• 
  

  Monday–Friday, 8:30 AM–5:00 PM

  
  


• 
  

  Some on-call, weekend, and holiday rotation

  
  

What You’ll Do

• 
  

  Provide one-on-one nursing care in patients’ homes

  
  


• 
  

  Perform initial and ongoing assessments of patient needs

  
  


• 
  

  Develop and update personalized plans of care

  
  


• 
  

  Communicate closely with physicians, caregivers, and families

  
  


• 
  

  Educate patients on managing their health and recovery

  
  


• 
  

  Document accurately and on time, following all home health standards

  
  

What We’re Looking For

• 
  

  Active RN License in Pennsylvania

  
  


• 
  

  Minimum 1 year of Home Health or Hospice experience

  
  


• 
  

  CPR Certification (required)

  
  


• 
  

  Reliable transportation and valid driver’s license

  
  


•
