Job Description - Registered Nurse RN Home Health



Registered Nurse (RN) – Home Health | Honesdale, PA | $38–$40/hr

Schedule



  • Monday–Friday, 8:30 AM–5:00 PM



  • Some on-call, weekend & holiday rotation



What You’ll Do



  • Deliver one-on-one skilled nursing care in patient homes



  • Perform initial assessments and ongoing evaluations



  • Develop & update individualized care plans



  • Collaborate with physicians, families, and caregivers



  • Educate patients on managing their health



  • Complete documentation accurately and on time



What We’re Looking For



  • Active RN License in Pennsylvania



  • Minimum 1 year Home Health or Hospice experience



  • CPR Certification



  • Reliable transportation & valid driver’s license



  • A compassionate, dependable nurse who takes pride in their work
