Home Health RN Registered Nurse – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate full-time Home Health RN Registered Nurse to provide skilled nursing care for patients in their homes throughout the Venice/South Sarasota county area (South Sarasota, Siesta Key, Gulf Gate, Osprey, Laurel, Nokomis, Venice and surrounding areas). In this full-time role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence. 

Locations: Home health visits in Venice/South Sarasota county area including: South Sarasota, Siesta Key, Gulf Gate, Osprey, Laurel, Nokomis, Venice and surrounding areas

Schedule: Full-Time, M-F 40 hours. Required weekend/holiday on-call rotations. PRN available if desired!

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals


• Develop, initiate, and revise individualized plans of care as patient needs evolve


• Deliver skilled nursing services and treatments as outlined in the plan of care


• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies


• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Graduate of an approved nursing program with an active Florida RN license


• Prefers BSN or MSN degree


• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred


• Strongly prefers experience using Homecare Homebase (HCHB) EMR


• Strong assessment, critical thinking, and patient education skills


• Willingness to participate in weekend and holiday on-call rotations
