Position Overview

The Registered Nurse for Skilled Visits/Infusion provides high-quality home health clinical services to patients in their homes. We’re seeking outgoing, energetic, and enthusiastic nurses who are passionate about delivering patient-centered care.

What You’ll Do

• 
  

  Provide skilled nursing care based on each patient’s individualized Plan of Care

  
  


• 
  

  Communicate changes in patient condition to the Director of Nursing and appropriate staff

  
  


• 
  

  Educate and counsel patients and families throughout the care process

  
  


• 
  

  Maintain accurate and compliant clinical documentation per local, state, and federal regulations

  
  


• 
  

  Participate in performance improvement initiatives and professional development

  
  


•

    Wound care / VAC therapy
