Registered Nurse (RN) / Licensed Practical Nurse (LPN) Home Health Care (Per Diem)

Hourly Pay: $52-$55 (RN) | $40-$42 (LPN)

Position Overview:

As a Per Diem Home Health Nurse, you will be providing essential care to clients in their homes, offering both medical support and emotional guidance. Your role will be pivotal in helping individuals manage their health conditions, maintain independence, and improve their quality of life. 

What You'll Do:

• Conduct detailed client assessments and document findings in clear, concise reports

• Monitor and record vital signs (blood pressure, temperature, pulse, etc.)

• Administer prescribed medications and monitor client responses

• Provide wound care and track healing progress

• Educate clients and families on health management and care routines

• Deliver compassionate palliative care to individuals with end-of-life needs

• Recommend and assist in implementing adaptive care tools to improve patient comfort

• Communicate openly with families to ensure patient care plans are followed

What We Need From You:

• Active NJ LPN or RN license

• CPR certification (American Heart Association preferred)

• Reliable transportation to conduct home visits

• Ability to work autonomously and efficiently follow established care plans

• Compassionate, patient-centered approach to care

• Strong communication skills, both with patients and team members

• Experience with advanced care (trach/vent care, G-tubes, ostomy care, etc.) is a plus

• Comfort working in pet-friendly homes

Preferred Qualifications:

• Familiarity with basic tech tools (Microsoft Word, Excel, Outlook)

• Previous home care or acute care experience
