As a Home Health Case Manager, you’ll deliver skilled nursing care to patients in Assisted Living Facilities (ALFs) and in their homes—empowering them to live healthier, more independent lives.

What You’ll Bring

• A passion for building strong relationships with patients and families


• The ability to work independently while collaborating with a supportive team


• Clear, compassionate communication skills


• A current Wisconsin RN license (or eligibility to obtain one)

Your Impact

• Serve as a Case Manager, coordinating individualized care plans


• Conduct comprehensive assessments and develop tailored treatment strategies


• Collaborate with patients, families, and interdisciplinary teams to achieve optimal outcomes


• Document care accurately, including progress notes and discharge summaries


• Educate patients and caregivers to promote safe, confident transitions
