Registered Nurse (RN) - Medical Coordination Supervisor
Grand Junction, CO
One Full-Time Position. 40 Hours per week.
Monday through Friday, 8:00 AM to 5:00 PM.
Hiring Range: $30.30 - $33.03 / hr. Depending upon education and experience.
This position will remain open until filled.
Minimum Requirements
Required:
• Active and valid Registered Nurse (RN) license.
• Two years’ experience in medical field.
Preferred:
• Minimum of one year of professional work experience with Intellectual and Developmental
Disabilities (I/DD).
• Minimum of one year of professional work experience in psychiatric care.
• Minimum of one year of professional work experience in geriatric care.

Position Summary
The Medical Coordination Supervisor is responsible for medical case coordination of all individuals receiving services, for all Ariel programs and locations, ensuring that individuals are receiving appropriate medical care.

Essential Duties and Responsibilities
1. The Nurse will work with case managers and Residential Providers to provide medical information, support and training around signs and symptoms of medical conditions, while providing oversight and division of job duties of medical team.
2. Effectively monitor delegated tasks.
3. Establish rapport and support services for individuals receiving services, Residential Providers, Staff, and Guardians.
4. Collaborate with Ariel Residential Case Managers on medical issues.
5. Coordinate with community-based medical facilities for ongoing care management and participate in hospital discharge planning meetings
6. Teach and/or oversee QMAP medication administration training.
7. Develop, review, and sign medical protocols and designations per regulations.
8. Review and sign Professional Medical Information Pages (PMIP) as appropriate.
9. Administer medical assessments as required per regulations.
10. Coordinate with pharmacies to ensure accurate medication delivery and enroll individual receiving services in care.
11. Consult on new client intakes regarding medical needs and protocols.
12. Provide onsite monitoring for individuals receiving services with medical needs.
Additional Duties and Responsibilities
1. Must work closely with other Ariel Programs and offices.
2. Performs other duties as assigned.
Requirements
Required Knowledge, Skills, and Abilities
1. The ability to effectively communicate verbally and in written format in both individual and group settings.
2. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
3. Basic computer knowledge including Microsoft Office Word and Excel.
4. Strong knowledge of the type of work client welfare agencies participate in.
5. Exercise independent judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations regarding department operations.
6. Ability to organize and present case material in a straightforward and concise written manner such that communication among treatment team members is clear concerning the client/family.
7. Knowledge of human development about major milestones of development, normal development parameters and commonly regarded devices to measure such.
8. Knowledge of community resources (social, public agency, and information) to assist families in accessing these resources.
9. Ability to provide reliable, insured transportation for self and others through the community served.
10. Ability to demonstrate consistent and reliable attendance as scheduled and requested.
Education and Experience
Required:
• Active and valid Registered Nurse (RN) license.
• Two years’ experience in medical field.

• Minimum of one year of professional work experience in psychiatric care.
• Minimum of one year of professional work experience in geriatric care.
Certifications/Licenses
Required:
• Current Registered Nurse (RN) License
• Valid Regular Drivers License, with acceptable driving record.
• Medical CPR, First Aid
Supervisory Responsibilities
1. Oversight of medical procedures and medications, and delegation of tasks.
2. Supervision of Medical Administrative Assistant.
Physical Demands
1. This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to two hours without a break.
2. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.
3. This position requires lifting regularly up to 10 pounds.
4. This position requires the ability to hear and see at a level which is adequate to ensure client and staff health and safety. Adaptive devices are acceptable.
5. This position will require daily driving of up to one hour per day and occasionally up to eight hours per day.
Work Environment
1. Exposure to typical climate-controlled office environment when in the office.
2. Exposure to all climates and weather conditions while working in the field.
3. Exposure to bloodborne pathogens and infectious disease is possible but limited.
4. Exposure to hazardous chemicals is possible but limited.
