Registered Nurse (RN) / Oncology Nurse Navigator

Position: Registered Nurse (RN) / Oncology Nurse Navigator\n\nDept: Cancer Center\n\nShift: Days / full-time; 80 hours per pay period\n\nFacility: This position is at Upper Valley Medical Center (UVMC). The Oncology Nurse Navigator is a registered nurse (RN) who partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes. The RN is responsible for seeing that cancer patients receive treatment services that are timely, appropriate and comprehensive. This is accomplished through identification of patients\u2019 needs and working collaboratively with a multi-disciplinary team across the in-patient and out-patient continuum of care. The RN supports patient/family education, monitors the recommended treatment plan, and sees that appropriate follow up occurs in the outpatient setting. The RN also monitors identified quality measures and works with the multi-disciplinary team to reach expected outcomes.
