Registered Nurse (RN) or Licensed Practical Nurse (LPN)

POSITION SUMMARY:

Reports directly to the Director of Medical Services. Responsible for patient care at a behavior health residential and outpatient level treatment facility. Responsible for rounds, administering medications, assessing clients, and all associated documentation.

DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

• Provides patient care as needed.

• Coordinates medical care with providers as needed.

• Maintains continuity among teams by documenting and communicating actions.

• Conducts new client intakes, assesses and evaluates client’s continuing needs.

• Administering medication passes, managing client prescriptions and medication changes.

SUPERVISORY RESPONSIBILITIES:

• None

WORK ENVIRONMENT:

• Behavioral Health Residential and Outpatient level treatment facility for substance abuse and co-occurring disorders.

PHYSICAL DEMANDS:

• Must have reasonable ability to stand, walk, sit, handle, feel, reach, climb or balance, stoop, kneel, crouch or crawl, talk, hear, taste, and smell. Must be reasonably capable of lifting 20 pounds.

WORK HOURS:

• Position can be Part Time / Full Time, Non-Exempt.

• May be required to work holidays, weekends and any shift within a 24-hour day.

• Variable hour shifts (typically 12 hour).

Requirements

QUALIFICATIONS, SKILLS AND KNOWLEDGE REQUIRED FOR POSITION:

• Must meet all Arizona requirements for Registered Nurse (RN) or Licensed Practical Nurse (LPN).

• At least one-year experience in an equivalent position.

• CPR/FA, CPI Training, Fingerprint and TB clearances.

• Must possess adequate verbal and written communication skills.

• Must possess basic computer skills and knowledge of EHR software.
