Registered Nurse RN - Universal Hospice Case Manager

Otterbein Universal Hospice is a not-for-profit, faith-based ministry that is a national leader and innovator in retirement living communities and long-term care.  Otterbein Universal Hospice is seeking a Hospice (RN) Case Manager, to assist in the growth of our Hospice ministry. Otterbein Universal Hospice compliments already strong independent senior services, outpatient capabilities and clinical facilities at Otterbein.

Shift: Full-Time (No Scheduled Weekends), PRN

 Responsibilities

Otterbein is seeking an enthusiastic and professional Hospice (RN) Case Manager. 

• This role is responsible for primary caseload.


• This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Home Health licensure laws and agency policies and procedures.  

Qualifications

• Education: Bachelor’s degree in nursing preferred. Graduate of approved nursing program


• Licensure/Certification: Valid Ohio Registered Nurse (RN) license


• Experience: A minimum of 1-year direct patient care experience in hospice care
