We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. This position requires expertise in OASIS assessments, providing patient-centered care, and ensuring the highest quality of service to our home health patients. The RN will work weekends, performing OASIS assessments and providing essential care to patients in their homes.

Key Responsibilities:

• 
  

  Perform OASIS assessments on new admissions, recertifications, and other required timepoints.

  
  


• 
  

  Provide skilled nursing care to patients, including wound care, medication management, and patient education.

  
  


• 
  

  Coordinate with the interdisciplinary team to develop and implement patient care plans.

  
  


• 
  

  Ensure proper documentation in compliance with OASIS standards and regulatory requirements.

  
  


• 
  

  Educate patients and families about disease management, medication, and preventive health measures.

  
  


• 
  

  Communicate with physicians, caregivers, and other healthcare professionals to ensure optimal patient care.

  
  


• 
  

  Participate in weekend on-call rotations as necessary.

  
  

A few must-haves for Home Health RN Case Managers:

• Graduate of an accredited nursing program and active RN license in Kansas


• CPR certification


• Knowledge of state and federal home health regulations


• Clinical proficiency in nursing specialty, strong communication skills and compassion
