Job Description

JOB SUMMARY:                                   

The Wound Ostomy Continence Nurse serves as a role model. consultant, change agent, and facilitator. Utilizes the nursing process in the delivery of quality patient care for patients with ostomies and complex wound and drainage problems, as well as in the prevention and management of pressure ulcers and/or other disorders of the skin.  Serves as a liaison for the physician, nursing staff, ancillary departments and external agencies to implement processing of the patient through the health care continuum. Supports the System and Nursing Strategic Plans.  Supports System and Nursing Strategic Plans, Relationship-Based Care, and Shared Leadership.

 

ESSENTIAL FUNCTIONS OF THE JOB:

  1. Utilizes the nursing process to provide comprehensive programs of care, instructions, and rehabilitation of ostomy patients partnering with patients and families to identify and prioritize needs and care.
  2. Assesses, plans, implements, coordinates and evaluates pressure ulcer management; develops, implements, and monitors preventive programs for patients at risk.
  3. Functions as consultant to staff for patients throughout the health system with disorders of the skin, complex drainage and wound problems, as well as those associated with incontinence.
  4. Participates in educational and training programs for Nursing Service personnel, students, patients and their families, and the community.
  5. Demonstrates an awareness of current trends in specialty that influence nursing practice and participates in nursing research.
  6. Serves as a patient and family advocate.
  7. Recognizes patient's rights by providing for confidentiality of patient information and demonstrates respect for patient's dignity.
  8. Supports Shared Leadership.
  9. Participates in Performance Improvement activities related to Pressure Ulcer Prevention and works with unit managers to develop plans of action.
  10. Collaborates with other members of the healthcare team to plan and coordinate care for patients with wound and ostomy needs.
  11. Participates in the evaluation of products related to wound, ostomy, and skin care and serves as a consultant to the Product Standards Committee.
  12. Maintains an attitude of cooperation and flexibility and assists staff members to do the same in order to promote interdepartmental relations and customer satisfaction.
  13. Seeks and supports opportunities for professional growth and development.
  14. Offers support and feedback to peers.  Applies principles of caring and healing in interaction with patients and families and with colleagues.
  15. Serves as coach and mentor for less experienced colleagues.
  16. Offers consistent and visible support to team members.
  17. Develops and maintains policies relevant to wound, ostomy, and skin care.
  18. Professional standards and current research/evidence-based practice drive decision-making.
  19. Strives to achieve excellence of service as described in the CARE Commitments.
  20. Responds to the following Shore Health System codes: Code Red (in the immediate area), Code Pink (when called), Code Yellow (when called).
  21. Responsible for honest behavior in all matters.  To the best of the employee’s knowledge and understanding, complies with all Federal and State laws and regulations.
  22. Maintains the privacy and security of all confidential and protected health information.  Uses and discloses only that information which is necessary to perform the function of the job.
  23. Performs related duties as assigned.

Qualifications

MINIMUM EDUCATION, EXPERIENCE, AND LICENSE/CERTIFICATION REQUIRED:

Bachelor’s degree in nursing required, Master's degree preferred.  Meets licensing requirements to practice registered nursing in the State of Maryland. Minimum of four years of medical/surgical nursing experience.  Certification by the Wound Ostomy Continence Nurses (WOCN) Society when eligible.  Understanding of current nursing practice trends.  Knowledge in the cognitive, physical, emotional and chronological stages of human growth and development. Exhibits a positive attitude, excellent communication skills, the ability to work productively under stress and displays a professional demeanor.  Requires the ability to prioritize work loads.
