Job Description - Registered Nurse (RN),LPN

Join our team as a Licensed Practical Nurse/Registered nurse at Fountain Of Hope Homecare services. We are seeking a dedicated individual to provide high-quality nursing care to our patients. As a Licensed Practical Nurse/Registered nurse, you will play a crucial role in delivering direct patient care, educating patients and their families on various aspects of care, and contributing to the development and evaluation of care plans.

Responsibilities:
- Provide direct nursing services to patients in accordance with established care plans.
- Educate patients and families on the patient's care, disease processes, and medications.
- Administer nursing care following prescribed protocols.
- Contribute to the development, implementation, and assessment of individualized care plans.
- Report and document patient conditions, including any signs or symptoms indicating changes in health status.
- Prepare detailed clinical progress notes for each day of care provided.
- Adhere to industry standards and best practices in nursing care.

Qualifications:
- Must hold a valid LPN/RN license
- Strong communication and interpersonal skills.
- Attention to detail and strong organizational skills.
