The RN Supervisor coordinates clinical care by assessing client needs, developing individualized care plans, and supervising direct care staff to ensure high-quality home service delivery and regulatory compliance.

Core Responsibilities

• 
  

  Clinical Assessments: Conduct and document comprehensive physical, psychosocial, and environmental assessments. Collaborate with physicians to develop and update care plans.

  
  


• 
  

  Supervision & Compliance: Oversee MF/MC, CNA, PCA, and Companion providers. Conduct on-site supervisory visits every 92 days (CNA/PCA) or 122 days (Companion).

  
  


• 
  

  Staff Oversight: Perform annual competency evaluations for field staff. Ensure adherence to infection control, safety protocols, and health screenings (e.g., TB/Hepatitis clearance).

  
  


• 
  

  Care Coordination: Act as a clinical liaison for families and providers; manage referrals, provide client/caregiver education, and interpret agency policies for the community.

  Quality Assurance: Maintain accurate service records, attend mandatory staff meetings, and uphold agency standards for training and safety.

  
  

Minimum Qualifications

• 
  

  Licensure: Current, unrestricted State RN license and CPR certification.

  
  


• 
  

  Education: Graduate of an accredited nursing program.

  
  


• 
  

  Experience: 2+ years of home health or nursing home experience within the last 5 years.

  
  


• 
  

  Logistics: Valid driver’s license, reliable vehicle, and current auto insurance.

  
  


• 
  

  Compliance: Ability to meet bonding criteria and pass annual TB screenings.

  
  


• 
  

  Skills: Strong verbal/written communication; 5-year verifiable employment history.

  
  

Preferred Qualifications

• 
  

  2+ years of home care experience with extensive assessment skills.

  
  


• 
  

  Familiarity with Joint Commission standards.

  
  


• 
  

  ACLS certification.
