Job Description - Registered Nurse-Palliative Care

Description

University Physicians Association is seeking a compassionate and dedicated Registered Nurse (RN) to join our Palliative Care team. This vital role supports patients and families facing serious illness by providing patient-centered care that focuses on relief from the symptoms, pain, and stress of cancer and related conditions. You will be part of an interdisciplinary team committed to improving quality of life for patients at all stages of their disease. This position will work closely with physicians to provide a seamless patient experience.

Key Responsibilities:

  • Deliver expert clinical care focuses on the relief of pain, nausea, fatigue, anxiety, and other complex symptoms associated with cancer/cancer treatment.
  • Provide telephone triage for acute symptom escalation and care coordination, assessing patient needs, offering guidance, and escalating issues to providers as necessary
  • Coordinate patient schedules and working with providers to optimize clinic flow and expedite patient care, including triaging “add-on” visits.
  • Collaborate closely with palliative care physicians and nurse practitioners to implement and adjust individualized care plans based on changing patient needs
  • Conduct regular assessments to evaluate treatment effectiveness, patient comfort,  and psychosocial well-being
  • Serve as a central point of contact for patients, caregivers, and referring providers
  • Educate patients and families about disease progression, symptom expectations,  treatment options, and end-of-life care
  • Document all clinical communications, interventions, and outcomes accurately and timely in the electronic health record (EHR)
  • Coordinate with other departments (oncology, radiation oncology, surgical oncology, hospice, social work, chaplaincy) to ensure seamless, whole-person care
  • Participate in interdisciplinary team meetings to support integrated care delivery

Requirements

  • Current  RN license 
  • Minimum 1–2 years RN outpatient experience preferred, preferably in oncology, hospice, or palliative care
  • Certification in Hospice and Palliative Nursing (CHPN) is a plus, or willingness to obtain
  • Strong interpersonal, critical thinking, and communication skills
  • Ability to work independently and within a collaborative team environment
