Your Role:

• Perform Start of Care (SOC) admissions and OASIS assessments
  
• Build care plans and introduce services
  
• Educate patients and caregivers
  
• Maintain accurate documentation and compliance
  

What We Look For:

• RN license (Minnesota)
  
• 1+ year of OASIS/SOC experience
  
• Reliable vehicle and active auto insurance
