Job Description - Apartment Program Coordinator


Department: Residential

Reports to: Associate Director of Operations

Position Type: Full-Time Exempt

Salary: Up to $40,000 (DOE)

POSITION

This role is part of an interdisciplinary team that includes nurses, behavior therapists, and public benefits administrators. Professionally plans, monitors, and coordinates operations of health and habilitation programs for consumers in community-based settings to ensure a high quality of life. This is an exciting opportunity to work with a team of highly qualified professionals in a flexible environment while making a difference.

RESPONSIBILITIES

  • Manages a caseload of consumers with intellectual and developmental disabilities who live in apartments.
  • Oversees all aspects of the consumer's care and ensures each experiences a high quality of life based on their abilities.
  • Carries out monitoring and quality assurance responsibilities in accordance with the organization's policies and procedures, personnel manual, and applicable laws and regulations.
  • Facilitates effective relationships between community care providers/team with timely planning, coordination, and conflict resolution, as needed.
  • Transports and accompanies consumers for grocery shopping, medical appointments, and/or other necessary appointments/tasks.
  • Assists in coordinating a variety of recreational activities to enhance social development and meaningful community integration
  • Attends site visits on a weekly/bi-weekly basis overseeing the priority needs of consumers.

MINIMUM QUALIFICATIONS

  • Associate's Degree or equivalent
  • At least 2 years of related experience and/or training

  • An equivalent combination of education and experience may be considered.

LANGUAGE SKILLS

Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, consumers, guardians, family members, and the general public.

REASONING ABILITY

Ability to solve practical and complex problems and critically reason through various concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to prioritize work needs to manage time effectively.

WORK ENVIRONMENT

The work environment is varied between remote work, office, and community locations including host homes and hospitals. This position requires a personal vehicle for transportation to effectively monitor and coordinate care. The home office for this position is located in Aurora, CO.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to reach with hands and arms and talk or hear. The employee frequently is required to drive. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus.
