What You’ll Do:
- Complete in-home assessments and create individualized care plans
- Coordinate and supervise care delivery for clients
- Conduct caregiver introductions and support initial shift setup
- Perform regular client check-ins and reassessments
- Support and train caregivers in the field and in the office
- Ensure all care is delivered in compliance with company and state standards
- Serve as a liaison for families and community partners
- Maintain up-to-date, accurate client records
What We’re Looking For:
- 2+ years of experience in home care, home health, or similar role
- Strong communication and leadership skills
- Attention to detail and excellent follow-up skills
- Reliable transportation and valid driver’s license
- Passion for working with seniors and individuals in need
- Comfortable using care management systems and documentation tools
