Position Title: Care Manager

What You’ll Do:

Complete in-home assessments and create individualized care plans

Coordinate and supervise care delivery for clients

Conduct caregiver introductions and support initial shift setup

Perform regular client check-ins and reassessments

Support and train caregivers in the field and in the office

Ensure all care is delivered in compliance with company and state standards

Serve as a liaison for families and community partners

Maintain up-to-date, accurate client records

What We’re Looking For:

2+ years of experience in home care, home health, or similar role

Strong communication and leadership skills

Attention to detail and excellent follow-up skills

Reliable transportation and valid driver’s license

Passion for working with seniors and individuals in need

Comfortable using care management systems and documentation tools
