Case Manager

Max Enterprise is looking to add a dedicated and experienced professional to our team as a Case Manager. The selected candidate will be an integral part of our healthcare team and will be responsible for coordinating and providing case management, tracking, and organizational services to patients as they move through the diagnosis and treatment processes.

Key Responsibilities:

• Act as a primary point of contact for patients, providing comprehensive care and serving as a liaison between patients, healthcare providers, and other relevant parties.
• Coordinate comprehensive, efficient, and timely patient care, including appointments, surgery, and treatment schedules.
• Ensure compliance with healthcare laws, regulations, and standards, maintaining patient confidentiality at all times.
• Perform admission and ongoing assessments of patients, evaluating their needs and crafting customized care plans.

The Case Manager will also monitor patient progress and make necessary adjustments to the care plan, ensure effective communication between all parties, and provide necessary education and support to the patients and their families.

Required Qualifications:

• Bachelor’s or Master’s degree in Nursing or related field.
• Minimum of five years of relevant experience in a healthcare setting, preferably in case management.
• Current state licensure as a Registered Nurse (RN).
• Strong understanding of medical terminologies, procedures, and healthcare operations.
• Exceptional coordination, organizational, interpersonal, and communication skills are required.
• Ability to work both independently and within a team, effectively handling multiple cases simultaneously.

Applicants should also have a thorough understanding of insurance matters, ethical concerns, and patient rights. Proficiency in common office productivity software and medical record-keeping systems is essential.
