Case Manager-LPN (Dallas, TX area)

About the Role:
XstremeMD is seeking a highly motivated and experienced Licensed Practical Nurse (LPN) Case Manager to support our growing team. In this remote role, you will coordinate care for injured or ill individuals, ensuring timely, appropriate, and cost-effective medical treatment. You will serve as a vital link between employees, medical providers, employers, and insurance carriers.

Key Responsibilities:

• Assess, plan, coordinate, and evaluate medical care for injured or ill individuals.
   
• Monitor case progress and facilitate communication between all involved parties.
   
• Promote recovery and safe return to work.
   
• Maintain detailed and timely case documentation.
   
• Support quality assurance by meeting performance metrics and participating in training and evaluations.

What We’re Looking For:

• Active and valid LPN license.
•  Minimum 2 years of clinical nursing experience.
•  ER/ICU background preferred.
• Bilingual preferred
• Case management or workers’ compensation experience a plus.
•  Strong communication, organizational, and computer skills (Word, Excel, internet-based platforms).
•  Must type at least 50 WPM and manage time effectively in a remote work setting.
