Position Summary
The Manager, Clinical Assessment and Care Planning provides leadership for and manages the assessment and care planning functions of the Unit. This position is responsible and accountable for all aspects of the Clinical Assessment operations, including ensuring that the care planning team is fully scheduled and engaged in ensuring the timely comprehensive clinical assessments for all assigned members. The manager is accountable for ensuring that assessments are comprehensive and of high quality and include medical, behavioral health, and social needs, long term services and supports (LTSS) as covered in the MDS-HC and Supplemental tools.
The Manager, Clinical Assessment and Care Planning ensures the following: all assessment nurses are fully scheduled, all reachable CCA members are assessed within specified time frame; reassessment MDSs are completed on time and to specified requirements with consistently high quality; members are assessed, and the member data is accurately entered handed off to CCA in a timely manner.

Essential Duties & Responsibilities
· Oversees all aspects of the operational and clinical work of the RN Nurse Assessors, including comprehensive MDS-HC and Supplemental Assessments
· Oversees quality of clinical work by assessment nurses
· Delivers training when necessary
· Tracks assessment completion rates and engages in continuous improvement process to meet productivity and quality goals as needed
· Individual will be responsible for making high-level clinical, programmatic, and operational management decisions
· Support teams and track Key Performance Indicators (KPI) related to clinical care for members

Bachelor's Degree in Nursing with current nurse licensure in good standing in Massachusetts

Physical Requirements: Ability to function in a busy clinical environment and to make home visits in a community based setting. Visual and hearing ability sufficient to comprehend written/verbal communication and perform physical exams and assessments of patients
