Clinical Pharmacist - West Coast

***Must be licensed in California.

The ConcertoCare Clinical Pharmacist is the expert in drug therapy for the local market. This may include working with pharmacies or providers outside of ConcertoCare or with health plan partners. They will maintain a patient panel for chronic condition disease management including HTN, diabetes, COPD, and CHF. Chronic condition disease management includes adjusting drug therapy under protocol.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Execution of pharmacist-led disease state management via collaborative practice agreement.
• Maintain patient panel for chronic condition disease management including diabetes, HTN, CHF, and COPD.
• Collaboration with providers/team to provide exceptional care to our patients.
• Lead medication safety program in assigned market.
• Organize training/education to other practitioners on pharmacy updates and best practices.
• Assist in decreasing Medication cost utilization and MLR
• Other duties as assigned.

QUALIFICATIONS

• Must hold an active, unrestricted pharmacist license in the state of practice.
• Will be asked to practice in multiple markets and become licensed in new markets as required by business growth .
• At least 3 years of pharmacy experience OR ASHP-accredited PGY1 Residency.
• Experience in ambulatory care, managed care, or other clinical setting.
• Strong written/oral communication, ability to interact effectively with multidisciplinary team.
• Motivated self-starter that takes initiative and follows through on tasks.
• Chronic condition disease management experience preferred.
• Bachelor's degree or PharmD
• Knowledge of managed care and Medicare/Medicaid a plus.
• Open to becoming licensed in additional states.

Base Salary/ Wage Range $145,000 to $160,000 with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
