Position Overview
The Collaborative Care Director is a senior clinical and operational leader responsible for overseeing and scaling Adjuvant's collaborative care programs across our partner networks. This individual will serve as the primary liaison between our clinical team and partner practices, ensuring care coordination models are implemented with fidelity, measured for outcomes, and continuously improved.
This is a fully remote position reporting directly to theVP of Operations and Strategy. The ideal candidate is a systems thinker who is equally at home coaching clinicians, sitting in strategy conversations with hospital administrators, building and refining program excellence, and diving into care quality data.
Key Responsibilities
Clinical Program Leadership
Oversee the design, implementation, and quality assurance of collaborative care programs across all partner sites
Provide clinical supervision and consultation to care managers, social workers, and behavioral health consultants embedded in partner practices
Ensure evidence-based protocols are followed for depression, anxiety, adjustment disorders, and other psycho-oncology presentations
Lead regular case consultations and team huddles with psychiatric consultants and care managers
Partner Engagement & Growth
Serve as the primary clinical point of contact for oncology practice partners, building strong relationships with medical directors, nurse navigators, and practice administrators
Support onboarding and site implementations with new and existing partnership expansions by training clinical staff and establishing workflow integrations, governance and accountability structure
Collaborate with the VP of Operations and Strategy and business development team to identify expansion opportunities within existing and prospective health system and community oncology partnerships
Operations & Outcomes
Monitor patient outcome data (PHQ-9, GAD-7, and other validated measures) and report on program performance to leadership
Develop and refine care protocols and clinical workflows in collaboration with our medical and operations teams
Contribute to grant reporting requirements, including state Rural Health initiatives and Medicaid population health metrics
Ensure compliance with telehealth regulations, payer requirements, and collaborative care billing standards
Team & Culture
Help foster a team culture defined by compassion, excellence, and accountability
Mentor clinical staff and support professional development pathways
Participate in leadership team meetings and contribute to organizational strategy
People Management
Management and oversight of all roles hired by ABH in accordance with the program.
This includes payroll, disciplinary actions, hiring, onboarding and continuing education to staff involved in CoCM delivery.
Qualifications
Master's or Doctoral degree in Social Work, Counseling, Psychology, or a related behavioral health field
Active, unrestricted licensure (LCSW, LPC, PhD, PsyD, or equivalent) in at least one U.S. state; ability to obtain licensure in additional states preferred
5+ years of clinical experience, with at least 2 years in a leadership, supervisory, or program management role
Demonstrated experience with collaborative care models (IMPACT, DIAMOND, or similar) or integrated behavioral health in medical settings
Strong understanding of cancer-related psychosocial needs and psycho-oncology best practices
Comfort working in a fast-growing, startup-style environment with evolving systems and processes
Preferred
Experience working within oncology, chronic illness, or specialty medical settings
Familiarity with Medicaid populations and value-based care arrangements
Experience supporting or managing telehealth programs across multiple states
Knowledge of grant-funded or state-supported rural health programs
Compensation & Benefits
Salary: $105,000 – $115,000 annually, commensurate with experience
