Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities, and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. With over 200 employees we strive to provide high-quality and innovative health care services to the Finger Lakes Region.

Position Summary
The Community Health Worker (CHW) serves as an advocate for the social, physical, and mental well-being of
underserved populations. This position works with agricultural workers and their families to overcome barriers to care through short-term care coordination, connection to resources, and support. Spanish proficiency is required for this position.



This position will work remotely, covering the Southern Tier region, and may occasionally require reporting onsite to our Bath, NY facility.








Main Responsibilities


Agricultural Worker Outreach



  • Conduct outreach to agricultural worker homes, worksites, and community locations to promote access to
    health and social services.

  • Build and maintain relationships with community organizations and partners serving the agricultural worker population.

  • Serve as the point of contact for clinical staff during mobile medical visits for services including:

    • Health screenings, immunizations, chronic care, reproductive health, etc.



  • Provide interpretation, appointment scheduling, paperwork assistance, and patient education during outreach
    and mobile events.

  • Educate agricultural workers on payment assistance programs and collect required documentation.



Patient Support



  • Identify socio-economic issues that affect patients' overall health and develop health/social management plans and goals.

  • Assist patients in accessing healthcare and social service resources to address identified barriers.

  • Support care coordination through appointment scheduling, form completion, and chronic condition management.

  • Provide interpretation services during in-person and telehealth visits and advocate for patients during care interactions.

  • Conduct follow-up communication to review care plans and ensure patient understanding.

  • Communicate with care team members to maintain alignment on patient updates, concerns, and care objectives.



Transportation



  • Inform patients via phone of scheduled pick-up time and location.

  • Coordinate or arrange transportation for medical appointments based on insurance eligibility, which may include direct patient transport when appropriate.



Documentation



  • Record patient encounters, activities, and patient outcomes in the electronic health record system.

  • Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures.



Other Duties



  • Perform other related duties as assigned by the supervisor.





Physical Requirements



  • Sitting/Standing: Prolonged sitting with occasional standing or walking.

  • Active Work: Frequent standing and walking, with occasional bending, crouching, or reaching.

  • Computer Use: Frequent typing and prolonged screen time.

  • Moderate Lifting: Regularly lifting or carrying items weighing between 15-40 pounds.

  • Visual: Clear vision for reading and inspections.

  • Hearing Clarity: Ability to hear conversations in person or in virtual communications.

  • Noise Sensitivity: Work may require filtering background noise to focus on speech or signals.



Work Environment



  • Health Center/Clinic: Work within a healthcare facility, interacting with patients and staff.

  • Outreach: Outreach work within community and field settings, engaging with underserved and at-risk communities.

  • Moderate-Paced: Dynamic environment that requires both productivity and accuracy to reach goals and project objectives.

  • Office Hours: Standard business hours, with occasional extended shifts based on clinic needs.

  • On-Call Hours: Outside of standard office hours, including evenings and sometimes weekends



Education



  • High school diploma or equivalent is required.

  • Associate or bachelor’s degree in public health, social work, human services or other related field is preferred.



Qualifications



  • 2+ years in a healthcare or customer service setting preferred.

  • Tested proficiency for communication in Spanish is required.

  • A valid government-issued photo identification document is required.

  • A valid New York State driver’s license in compliance with FLCH driver’s policy is required.

  • Current New York State automobile insurance in compliance with FLCH underwriting agreement is required.

    • Policy limits should reflect $100,000/$300,000/$100,000 coverage.



  • Certification in medical or community interpretation from a recognized accrediting body (e.g. CCHI, NCMI) in
    accordance with FLCH policy is required.

  • Applies working knowledge of community health principles in outreach and support programs.

  • Able to collaborate across roles to achieve shared outcomes.

  • Uses judgement to adapt communication style to various audiences and situations.

  • Applies sound reasoning while analyzing problems and implementing effective solutions.Community Health Worker.
