Position Responsibilities may include, but not limited to
- Build and maintain strong relationships with key community stakeholders, including social workers, discharge planners, senior centers, non-profits, and other community locations.
- Promote Mom’s Meals products and products through in-person outreach, community events, presentations, and educational sessions.
- Identify and engage eligible clients, assisting them with understanding their options and navigating the enrollment process.
- Act as a liaison between clients, referral sources, and internal teams to ensure seamless service delivery.
- Track outreach activities, client engagement, and outcomes using CRM tools or internal systems.
- Stay up to date on local and state programs that offer meal benefits and collaborate to align services.
- Represent Mom’s Meals at local fairs, expos, conferences, and coalition meetings.
- Provide feedback from the field to improve customer experience and address barriers to access.
- Offer and schedule onsite (preferred) or virtual in-services with target organizations
- Be highly visible within PA communities by engaging in community-based events
- Attend and participate in county round tables and county and state webinars to represent Mom’s Meals
- Cover for Manager of Healthcare Partnerships when needed
Required Skills and Experience
- Associate’s or Bachelor’s degree
- 2-4 years' community outreach, sales, or healthcare
- Demonstrated ability to successfully engage community health organizations
- Proven ability to work with cross functional teams
- Excellent oral and written communication skills; experience giving presentations
- Experience using Microsoft Office suite, including PowerPoint and Excel
- Willingness to travel locally and statewide; travel 50-75%
- Must possess a valid driver’s license and reliable transportation
Preferred Skills and Experience
- Experience in community outreach, sales, social services, or healthcare
- Strong interpersonal and communication skills, with a customer-focused approach
- Comfortable working independently in the field with minimal supervision
- Passion for serving underserved or vulnerable populations
- Knowledge of Medicaid, Medicare, and social assistance programs
Physical Requirements
- Repetitive motions that include the wrists, hands and/or fingers
- Sedentary work that primarily involves sitting, remaining in a stationary position for prolonged periods
- Visual perception to perform job including peripheral vision, depth perception, and the ability to adjust focus
