Child and Family Connection #15, a program of Service, Inc. of Illinois, invites you to apply for our open Early Intervention Service Coordinator position supporting families to help their children under age 3 to meet developmental milestones.
This position offers flexible Telework options and supports work-life balance with a generous PTO benefit program and a starting salary range of $40,000.00 to $52,000.00 annually (based on prior related work experience).
We are seeking a high energy, organized, compassionate person to join our agency team of nearly 200 professionals.
As a non-for-profit organization, Child & Family Connection employees may be eligible for federal student loan forgiveness programs.
Qualified candidates must reside within a reasonable commute distance from our Joliet or Kankakee office and your home location to meet caseload visiting needs.

As a Service Coordinator (SC) in our Early Intervention Program, your Essential Job duties include, but are not limited to:
Provide families with information about the EI Services System, including information regarding choices and options of available services and supports to meet the child’s needs
Work collaboratively as a team with the family and service providers to support children enrolled in the EI Program.
Coordinate and ensure completion of initial and annual evaluations and assessments
Facilitate and participate in developing the Individual Family Services Plan (IFSP) to support the child and assist the family in monitoring IFSP implementation and progress
Provide ongoing advocacy support to ensure that appropriate EI services are being provided by communicating monthly with the family, using a variety of mechanisms, including face-to face visits
Maintain accurate documentation in the form of case notes
Facilitate the development of a transition plan to preschool, school, or if appropriate, to other services.
Requirements
Bachelor’s Degree in Human Services or a related field (Required)
Birth to age 5 Case Management experience (Preferred)
Minimum one-year experience working with IDD and/or disabled population (Preferred)
Early Intervention Credentialed (Preferred)
Early Intervention Credential (Completion Required within 3 months of hire date)
Valid Driver’s License, Insurance, and reliable transportation (Required)
Ability to meet Essential Job Functions (with or without reasonable accommodation) (Required)
