POSITION DESCRIPTION



JOB TITLE: Family Nurse Practitioner


Days of operation: Monday-Saturday (flexible schedule)
Hours of operation: 8 AM-6 PM 
Location: Halsey Plaza NE Portland, Oregon 97220
Rate: 1-3 years $125-$140 a year starting 
Rate: 4-7 years working independently, $141-160 a year 
Full-Time 

 



JOB SUMMARY:


The Family Nurse Practitioner (FNP) specializes in providing comprehensive healthcare services to individuals and families across their lifespan. FNPs are trained to assess, diagnose, and treat a wide range of medical conditions.



FUNCTIONS OF THE JOB:


Here, our FNP will be able to utilize their full scope working independently as an FNP, which includes a licensed healthcare professional who provides a wide range of healthcare services:


Diagnosis and management of acute, chronic, and complex health problems; health promotion; disease prevention; health education; and therapeutic services, counseling to individuals, groups. In addition, specialize in delivering primary care services to individuals and families. Responsible for assessing, diagnosing, and treating a variety of acute and chronic medical conditions, promoting health and wellness, and providing patient education. 


BMWIT FNPs must be willing to support mental health, providing therapeutic services and patient assessments, medication management, prescribing and discussing their mental health conditions with them and the treatment care team. FNPs will also perform case management on all established patients, as well as close a patient case. This role requires collaboration with other healthcare providers internally and externally, and adherence to established guidelines and standards of practice.



KEY RESPONSIBILITIES: 


Patient Assessment and Diagnosis

•        Conduct comprehensive assessments of individuals and families by discussing their health and mental health conditions with them.


•        Ordered and interpreted diagnostic tests to aid in diagnosing medical conditions.
•        Formulate differential diagnoses based on assessment findings.
•        Treatment and Management:
•        Develop and implement evidence-based treatment plans for acute and chronic illnesses.
•        Prescribe medications and treat clients to ensure medications are correctly administered.


•        Monitor patient progress and modify treatment plans as needed.


Health Promotion and Disease Prevention

•        Provide evidence-based interventions and coordinate treatment/care plans for clients. Provide education on health maintenance, disease prevention, and lifestyle modifications.


•        Conduct routine screenings and immunizations.
•        Collaborate with patients to create personalized wellness plans.

 

Collaboration and Communication

•        Collaborate with physicians, nurses, and other healthcare and mental health professionals to ensure coordinated and comprehensive care.
•        Communicate effectively with patients, families, and the healthcare and clinical team.
•        Participate in interdisciplinary meetings and case conferences.
 

Record Keeping and Documentation

•        Maintain accurate and up-to-date electronic health records.
•        Document patient encounters, assessments, diagnoses, and treatment plans in compliance with regulatory standards.


Quality Improvement

•        Participate in quality improvement initiatives to enhance patient care outcomes.
•        Stay current on evidence-based practices and incorporate them into clinical practice.
•        Continuing in Educational Training
