Remote MTM/Med Adherence Pharmacist - Dual State Licensing Required - Unlimited Growth Potential

Soliant Health is seeking a Remote MTM/Med Adherence Pharmacist for a 5-6 month contract. This pharmacy team plays a pivotal role in bridging the gap between practices and insurance panies. This role involves direct patient outreach, working closely with Medicare patient populations. Must be dual licensed in Louisiana, New Jersey, Georgia, or Kentucky.

• Position:Remote MTM/Med Adherence Pharmacist
• Start Date:ASAP
• Duration: 5 Months
• Hours:Monday - Friday, 9:00 am - 5:00 pm EST (flexibility required as hours may expand)
• Location: Work from Home - Fully Remote

Duties:

• Conduct remote medication adherence/management calls, addressing elevated inquiries that technicians cannot legally handle.
• Prevent, detect, and resolve medication-related issues, offering evidence-based, patient-centered remendations.
• Perform thorough medication reconciliations for post-discharge patients.
• Assist in managing high-risk, high-cost patients to ensure optimal care.
• Conduct telephonic prehensive medication reviews (CMRs) for qualified patients.
• Identify and address patient-specific barriers to medication adherence and disease management.
• Provide clinical decision support, education, and consults to providers as needed.
• Enhance care coordination among care managers, providers, and pharmacies.
• Collaborate with providers and staff to optimize Medication Management workflow and patient experience.
• Adjust and remend alternative pharmacotherapy based on formulary changes.
• Follow evidence-based guidelines for managing chronic disease states.

Experience Required:

• Strong MTM background.
• Ideal candidate will have experience as a Health Plan or PBM pharmacist, or similar roles performing these duties.

Licensure:

• Must hold at least TWO of the following licenses: LA, NJ, GA, KY (No Exceptions)

Requirements for Working from Home:

• Private workspace with desk and chair, minimal background noise.
• Minimum internet speed: 100mbps download, 10mbps upload.
• Ethernet connection required (no Wi-Fi).
