• Respond to patient and provider support requests and communications for all service lines via in-person, phone or electronic communication.
• Communicate with providers to ensure collaboration among members of the care team prioritizing quality, integrated care.
• Match new and current patient service requests based on criteria set for patient-provider matching such as reason for seeking treatment, appropriateness for care, insurance compatibility, and provider availability.
• Monitor patient completion of documentation requirements to ensure compliance with organizational policy and procedure.
• Perform patient care support tasks related to scheduling, records requests and releases of information, patient information updates (e.g., demographics, contact info), documentation assignments, medication refills, and technology troubleshooting.
• Collaborate and support other care coordinators to ensure the care team prioritizes quality, integrated care.
• Report qualitative and quantitative information to Patient Care Specialists and Care Manager to support ongoing feedback of organizational processes.
• Provide trauma-informed support by responding to team members or patients with empathy and professionalism.
• Comply with all relevant federal and state laws, regulations, and ethical guidelines related to mental healthcare such as but not limited to the HIPAA Privacy Rule, the Mental Health Procedures Act, and the Health Information Technology for Economic and Clinical Health Act.
• Support a positive and collaborative work environment with team members.
