Position Summary
The Remote Patient Monitoring (RPM) Nurse / Program Coordinator is responsible for the day-to-day operation, coordination, monitoring, and ongoing development of Kane County Hospital’s Remote Patient Monitoring and Digital Care Program.
This position serves as the primary operational lead for the RPM program and works collaboratively with Kane County Hospital providers, clinic staff, Information Technology, billing and finance personnel, and technology partners to ensure the program is implemented effectively and operates safely, efficiently, and consistently.
The RPM Nurse / Program Coordinator is expected to take initiative, identify needs and barriers, develop solutions, follow issues through to resolution, and maintain ownership of program operations. The position requires a highly organized, self-directed nurse who is comfortable building new workflows, learning new technology, communicating with patients and providers, tracking performance, and independently managing competing priorities.
The Operations Director will provide program oversight. ChartSpan and other implementation partners will provide technical assistance, education, and guidance as the program is established and refined.
Essential Duties and Responsibilities
RPM Program Management
Coordinate and manage the day-to-day operations of the Remote Patient
Monitoring Program.
Serve as the primary internal point of contact for RPM program questions,
workflows, implementation needs, and operational concerns.
Take ownership of program implementation and ongoing operations with oversight
from the Operations Director.
Work with ChartSpan and other technology partners during implementation,
training, troubleshooting, and workflow development.
Develop, maintain, and improve standardized RPM workflows, procedures, and
operational processes.
Identify workflow problems or program barriers and proactively develop solutions.
Follow identified issues through resolution rather than waiting for direction on
routine operational matters.
Maintain organized records related to program enrollment, devices, monitoring,
clinical interventions, performance measures, and required reporting.
Coordinate meetings and communication among providers, clinic staff, technology
partners, IT, billing, and hospital leadership as needed.
Keep the Operations Director informed of program progress, significant concerns,
barriers, and opportunities for improvement.
Patient Identification and Enrollment
Work with providers and clinic staff to identify patients who may benefit from
Remote Patient Monitoring.
Initially support RPM enrollment for patients with conditions such as hypertension
and diabetes, with expansion to additional populations as the program develops.
Support future expansion to patients with congestive heart failure, COPD, postdischarge
monitoring needs, and other eligible conditions as approved.
Review referrals and verify that patients meet established program eligibility
requirements.
Contact eligible patients and explain the RPM program.
Obtain and document appropriate patient consent.
Enroll patients into the RPM platform according to established procedures.
Coordinate distribution and activation of monitoring devices.
Educate patients and caregivers on device use, monitoring expectations,
communication procedures, and when to seek urgent or emergency care.
Provide additional support for patients with limited health literacy, digital literacy,
transportation access, or technology experience.
Patient Monitoring and Clinical Oversight
Routinely review patient-generated health information received through the RPM
platform according to established monitoring standards.
Monitor data such as blood pressure, blood glucose, weight, oxygen saturation, and other approved measurements based on the patient's condition and care plan.
Identify readings, trends, symptoms, or changes that fall outside established
clinical parameters.
Contact patients as appropriate to obtain additional information and complete
nursing assessment within the RN scope of practice.
Provide patient education and reinforce established care plans.
Document monitoring activities, patient communication, interventions, and
escalation in accordance with KCH standards.
Promptly communicate clinically significant findings to the appropriate provider.
Follow established escalation protocols for abnormal readings or concerning
patient symptoms.
Track provider recommendations and assist with ensuring appropriate follow-up
occurs.
Recognize situations requiring urgent or emergency evaluation and direct patients
according to established clinical protocols.
Maintain clear boundaries between nursing monitoring and provider medical
decision-making. Providers remain responsible for diagnosis, treatment changes,
prescribing, and other medical decisions.
Provider Communication and Follow-Up
Serve as a communication link between RPM patients and their healthcare
providers.
Provide providers with concise, relevant information regarding abnormal readings,
patient concerns, and meaningful trends.
Track escalated concerns to ensure appropriate follow-up has occurred.
Assist with coordinating follow-up appointments, lab work, imaging, medication
review, or other services ordered by the provider.
Communicate recurring trends or concerns that may warrant adjustment of the patient's monitoring plan.
Work collaboratively with Medical Assistants, nurses, registration staff, and other
clinical personnel to prevent gaps in patient follow-up.
Device and Technology Coordination
Maintain knowledge of the RPM platform and approved monitoring equipment.
Coordinate device assignment, distribution, activation, replacement, and return.
Maintain accurate device inventory and patient assignment records.
Assist patients with basic troubleshooting and coordinate higher-level technical
support through ChartSpan or other technology partners when needed.
Monitor unsuccessful or missed data transmissions and follow up according to
established procedures.
Work with IT and technology partners to identify and resolve system or connectivity
concerns.
Participate in testing and optimization of Cerner CommunityWorks integration and
RPM documentation workflows.
Program Tracking and Reporting
The RPM Nurse / Program Coordinator will maintain accurate program data and routinely
track measures including:
Patients identified for RPM
Provider referrals
Patients contacted
Enrollment acceptance and decline rates
Active patient enrollment
Device activation
Successful data transmission
Missed transmissions
Patient retention
Monitoring activity
Abnormal readings and escalations
Clinical interventions
Completed follow-up
Device utilization
Technical support needs
Patient satisfaction
Program barriers
Emergency department visits, hospitalizations, or readmissions when applicable
Other required grant and quality measures
The RPM Nurse / Program Coordinator will assist the Operations Director and other KCH
leadership with preparation of required program reports and performance information.
Grant and Quality Responsibilities
Support implementation of the LIFT Initiative Remote Patient Monitoring and Digital
Care project.
Assist with collection of required grant performance measures and supporting
documentation.
Maintain accurate records to support program monitoring, audits, and grant
reporting.
Participate in quality-improvement activities related to the RPM program.
Compare program performance with established baseline measures and project
goals.
Identify trends, gaps, and opportunities for program improvement.
Help evaluate whether the RPM program is reaching patients with significant rural
access barriers.
Participate in program reviews with hospital leadership and implementation
partners.
Assist with preparing for grant monitoring activities or site visits as requested.
Billing and Sustainability Support
Work with billing, finance, providers, and leadership to establish documentation
workflows that support compliant RPM billing.
Ensure required clinical activities and patient interactions are appropriately
documented.
Assist with identifying operational issues that could affect billing or reimbursement.
Participate in monitoring patient participation, program utilization, and operational
efficiency.

Assist leadership in evaluating opportunities to expand the program as patient
needs, reimbursement, and organizational capacity evolve.

and other staff involved in RPM workflows.
Provide ongoing education as workflows change or new patient populations are
added.
Develop or maintain patient education materials and staff reference resources.
Reinforce consistent use of approved workflows, documentation standards, and
escalation procedures.

The RPM Nurse / Program Coordinator is expected to:

Qualifications
Required:
Current Registered Nurse license in the State of Utah or compact-state eligibility as
applicable.
Current BLS certification.
Strong clinical assessment and communication skills.
Strong organizational skills and attention to detail.
Ability to work independently and manage multiple priorities.
Demonstrated initiative, dependability, and follow-through.
Ability to communicate professionally with patients, providers, leadership, and
outside partners.
Ability to learn and effectively use electronic health records, remote monitoring
platforms, and other healthcare technology.
Ability to recognize changes in patient condition and appropriately escalate clinical
concerns.
Preferred:
Experience in outpatient nursing, chronic disease management, care coordination,
population health, case management, or remote patient monitoring.
Experience caring for patients with hypertension, diabetes, CHF, or COPD.
Experience with Cerner CommunityWorks or similar electronic health record
systems.
Experience implementing a new clinical program or workflow.
Experience with quality improvement, data tracking, grant-supported programs, or
healthcare program coordination.

Reporting Structure
The RPM Nurse / Program Coordinator reports to the Operations Director, who provides
overall administrative and operational oversight of the program.
The RPM Nurse / Program Coordinator works collaboratively with KCH providers regarding
clinical patient-care decisions. The nurse may identify, assess, document, and escalate
concerns within the RN scope of practice; however, providers remain responsible for
medical diagnosis, medication changes, treatment decisions, and determination of higher
levels of care.
ChartSpan and other approved implementation partners will serve as resources for
implementation guidance, platform support, workflow development, training, and
troubleshooting as appropriate.
