We are seeking a CertifiedPeer Recovery Specialist Lead to support our members in the Substance Use Disorder Program.

Our Peer Recovery Leads support a team of Peer Recovery Specialist's in following the Substance Use Disorder Program guidelines in planning care, assessing member needs, providing resources and serving as role models for individuals facing substance-exposed living. In this position you also have the flexibility and autonomy to work from home and in the community if needed.

Some of the responsibilities include:
Partner with leadership and the enrollment team to grow the census and identify eligible members to enroll in the program.
Act as a player/coach for a team of peers while modeling and exemplifying the company core values.
Lead and manage a team of peers with the responsibility of overall team success – includes, but is not limited to, driving alignment with operations and clinical team
Manage day to day operations of the peers including regular 1:1s, team meetings, and case conferences.
Engage in ongoing performance management and coaching of team members to ensure all issues are addressed and resolved in a timely manner, including but not limited to, disciplinary action as necessary.
Maintain a small caseload and provide back up to peers for clinical care during PTO, staff shortages, etc.
Conduct meaningful outreach, provide education, and drive member enrollment
Collaborate with member, family and healthcare providers to develop an individualized recovery action plan.
Identify and initiate referrals for social service programs – including financial, psycho-social, community and state supportive services.
Complete documentation for client engagements, cancellations and care plan.
Provide support to clients through a variety of settings and assists with support groups, as needed.
Advocate for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the healthcare team
Represent and promote the Substance Use Disorder Program/Mindoula through participation and interaction with community, professional organizations and community outreach efforts.
Achieve set goals; Key Performance Indicators (KPIs).
Participate in interdisciplinary team meetings and utilization management rounds and provide information to assist with safe transitions of care.
Provide peer recovery support through the on-call crisis line.
Promote responsible and ethical stewardship of company resources.
Maintains excellent punctuality and attendance during work hours.
Other duties as assigned.
Requirements:
You must be Certified in Peer Recovery in one of the following states: TN, TX, LA, KY, IN, FL, MO or WA.
You must have at least 1-2 years of management/leadership experience to be considered for this position.
This position is 100% Remote.
