Job Description - Peer Recovery Specialist Lead

We are seeking a Certified Peer Recovery Specialist Lead to support our members in the Substance Use Disorder Program.

Our Peer Recovery Leads support a team of Peer Recovery Specialist's in following the Substance Use Disorder Program guidelines in planning care, assessing member needs, providing resources and serving as role models for individuals facing substance-exposed living. In this position you also have the flexibility and autonomy to work from home and in the community if needed.

Some of the responsibilities include:

  • Partner with leadership and the enrollment team to grow the census and identify eligible members to enroll in the program.
  • Act as a player/coach for a team of peers while modeling and exemplifying the company core values.
  • Lead and manage a team of peers with the responsibility of overall team success – includes, but is not limited to, driving alignment with operations and clinical team
  • Manage day to day operations of the peers including regular 1:1s, team meetings, and case conferences.
  • Engage in ongoing performance management and coaching of team members to ensure all issues are addressed and resolved in a timely manner, including but not limited to, disciplinary action as necessary.
  • Maintain a small caseload and provide back up to peers for clinical care during PTO, staff shortages, etc.
  • Conduct meaningful outreach, provide education, and drive member enrollment
  • Collaborate with member, family and healthcare providers to develop an individualized recovery action plan.
  • Identify and initiate referrals for social service programs – including financial, psycho-social, community and state supportive services.
  • Complete documentation for client engagements, cancellations and care plan.
  • Provide support to clients through a variety of settings and assists with support groups, as needed.
  • Advocate for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the healthcare team
  • Represent and promote the Substance Use Disorder Program/Mindoula through participation and interaction with community, professional organizations and community outreach efforts.
  • Achieve set goals; Key Performance Indicators (KPIs).
  • Participate in interdisciplinary team meetings and utilization management rounds and provide information to assist with safe transitions of care.
  • Provide peer recovery support through the on-call crisis line.
  • Promote responsible and ethical stewardship of company resources.
  • Maintains excellent punctuality and attendance during work hours.
  • Other duties as assigned.

Requirements:

  • You must be Certified in Peer Recovery in one of the following states: TN, TX, LA, KY, IN, FL, MO or WA.
  • You must have at least 1-2 years of management/leadership experience to be considered for this position.

    This position is 100% Remote.
