We are currently seeking a Primary Care Nurse Practitioner (APRN) to serve as a float provider within our outpatient system in Boca Raton, FL. This is a full-time position with a consistent weekday schedule, offering the opportunity to work across multiple primary care locations as needed.

The float APRN will play a critical role in delivering high-quality, evidence-based care to a diverse patient population, supporting continuity of care and helping ensure consistent access across our clinical sites.

Position Summary:

The Primary Care APRN will provide patient-centered care through face-to-face visits, telehealth, and other communication platforms. This role includes comprehensive clinical assessments, diagnosis, treatment planning, and patient education. Working collaboratively with physicians and the care team, the APRN will help lead clinical efforts to improve patient outcomes and enhance system-wide care delivery.

Qualifications:

• Current and active APRN license in the state of Florida.
  

• Primary care experience is strongly preferred.
  

• Ability to adapt quickly to different clinic environments and patient populations.
  

• Proficient in the use of EMR systems and telehealth platforms.
  

Key Responsibilities:

• Provide primary care services across various outpatient settings as a float provider.
  

• Conduct patient assessments, including H&Ps and thorough medical histories.
  

• Order and interpret diagnostic and laboratory tests.
  

• Develop medical diagnoses and implement individualized treatment plans.
  

• Prescribe medications and perform approved procedures per Delineation of Privileges.
  

• Deliver care through telehealth and other virtual platforms when appropriate.
  

• Educate patients and families on diagnoses, treatment plans, and preventive care.
  

• Collaborate with physicians, nursing staff, and administrative teams to ensure quality care.
  

• Manage acute and deteriorating patients when necessary.
  

• Integrate evidence-based practices and new technologies into daily practice.
  

• Participate in discharge planning, care coordination, research, and quality improvement initiatives.
