Remote Psych NP (Telehealth) Long Island NY

We are seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to join a growing outpatient telehealth practice focused on delivering high -quality psychiatric care to adult and geriatric populations. This is a fully remote position supporting a New York–based clinic, and providers must be located within the Eastern or Central time zones with the ability to work Eastern Time hours.

This opportunity is designed for full -time clinicians who are looking to build a consistent and meaningful outpatient practice while working from home. Providers will manage an established patient population with ongoing care needs, allowing for continuity, clinical impact, and long -term patient relationships.

Requirements

• Conduct comprehensive psychiatric evaluations and ongoing follow -up visits via telehealth
  
• Diagnose and manage a wide range of psychiatric conditions in adult and geriatric patients
  
• Develop individualized treatment plans that account for psychiatric and underlying medical conditions
  
• Prescribe and manage psychotropic medications with careful monitoring and follow -up
  
• Address complexities related to aging, comorbid medical conditions, and polypharmacy
  
• Maintain continuity of care through consistent patient engagement
  
• Collaborate with clinical and administrative teams to support coordinated outpatient care
  
• Provide education to patients and families regarding diagnoses, medications, and treatment plans
  
• Maintain accurate, timely, and compliant documentation within the EMR
  
  

• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
  
• Active, unrestricted New York State Nurse Practitioner license
  
• Ability to work full -time aligned with Eastern Time clinic hours
  
• Experience treating adult populations with comfort managing geriatric patients
  
• Strong understanding of how medical comorbidities impact psychiatric treatment planning
  
• Prior outpatient or telehealth experience preferred
  
• Ability to manage a consistent outpatient caseload independently
  
• Strong clinical judgment and decision -making skills
  
• Excellent communication and virtual patient engagement skills
  
• Comfort with telehealth platforms and EMR systems
  
• Spanish speaking preferred
