Position Overview
Paragon Behavioral Health Connections is seeking a compassionate and community-focused ACT Care Manager to deliver high-quality, client-centered behavioral health and case management services in Broomfield and Adams County. This key frontline role supports adults with complex needs through Assertive Community Treatment (ACT)-informed practices, including crisis intervention, integrated care, and housing navigation. The Care Manager collaborates within a multidisciplinary team to provide services directly in the community and client homes, promoting independence and long-term stability. Applicants must hold a bachelor’s degree in a behavioral health-related field and have relevant experience in behavioral health or case management.
This role reports to the Team Lead and operates as part of a shared caseload model.

Position Summary:
The ACT Registered Nurse plays a key role in delivering comprehensive, community-based psychiatric and medical care to individuals with serious and persistent mental illness, often co-occurring with substance use disorders. This role involves providing direct nursing services, managing medications, including those used in Medication-Assisted Treatment (MAT)—facilitating therapeutic groups, supporting case management, and collaborating with a multidisciplinary team to promote recovery, wellness, and successful community integration.
Key Responsibilities:
- Conduct comprehensive health assessments and monitor clients’ physical health conditions.
- Administer and manage medications for mental health and substance use disorders, including long-acting injectables and Medication-Assisted Treatment monitor for side effects and treatment effectiveness.
- Facilitate psychoeducational and therapeutic groups focused on health, wellness, recovery, and harm reduction.
- Provide education to clients and families on medications, chronic health conditions, self-care strategies, and wellness planning.
- Collaborate with psychiatric providers, primary care clinicians, mental health professionals, and substance use treatment specialists to ensure integrated, coordinated care.
- Participate in daily team meetings and contribute to individualized treatment planning.
- Conduct home visits and community outreach to support client engagement, medication adherence, and continuity of care.
- Maintain accurate, timely, and compliant documentation in accordance with agency policies and regulatory standards.
- Support crisis intervention, de-escalation, and stabilization efforts in community settings.
- Promote trauma-informed, recovery-oriented, and culturally responsive care practices.
- Coordinate with pharmacies and complete prior authorizations for medication approvals; assist providers with Prescription Drug Monitoring Program (PDMP) compliance.
- Under the direction of a medical provider, enter laboratory orders, track test completion, and follow up to ensure results are reviewed and addressed.
- Assist with provider scheduling and triage client needs to appropriate team members.
- Conduct mental health screenings (e.g., PHQ-9, GAD-7, PCL-5) and report abnormal findings to medical providers.
- Perform basic health screenings, including vital signs, urine drug screens, pregnancy tests, and other point-of-care testing.
- After completing Colorado-specific training, be prepared and willing to initiate an M1 hold when necessary to ensure client safety.
- Perform other duties as assigned to support team operations and client care.
Qualifications
Required:
- Active Registered Nurse (RN) license in the state of Colorado
- Valid driver’s license and access to reliable transportation
- Current CPR certification
- Up-to-date vaccinations in accordance with agency and public health guidelines
- A minimum of one to two years of experience working with individuals diagnosed with serious and persistent mental illness.
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience in community-based, psychiatric, or integrated care settings
- Familiarity with the Assertive Community Treatment model and trauma-informed care principles
- National certifications such as Psychiatric-Mental Health Nursing (PMH-BC) or Certified Addictions Registered Nurse (CARN)

Total Compensation:
Salary: $82,576-$93,600 annually
