We’re hiring a full-time, remote Nurse Practitioner to support Medicare and Medicaid members. You’ll focus on Medicare Annual Wellness Visits, risk adjustment, and HEDIS care gap closure — all from home, Monday through Friday.

  Mix of preventive and risk-based virtual visits

  Medicare Annual Wellness Visits

  Document risk assessments

  Close HEDIS care gaps

  
  


• 
  

  Handle HCC, ICD-10, and CPT II coding

  
  

• 
  

  Active NP license in IN (required), AL (required), and WA (required)

  
  


• 
  

  1+ year in Family or Internal Medicine

  
  


• 
  

  Medicare and Medicaid experience

  
  


• 
  

  Ready to work across Medicare, Medicaid, and Commercial lines

  
  


• 
  

  Medicare/Medicaid enrollment required
