Transitional Support Specialist

• Ensure that youth and foster families are connected to services that support the transition to independent living, including housing, education, employment, and healthcare.
• Provide training on life skills and treatment modalities to foster youth and their foster families to prepare them for the transition process.
• Track the progress of each youth’s transition plan, ensuring that goals are met, and provide regular updates to the team.
• Educate foster families and youth about eligibility for support programs, housing assistance, and other resources available after aging out of the system.
• Work closely with case managers, other specialists, and external service providers to ensure a smooth transition for youth.

• Bachelor’s degree in human or social services.
• Two (2) years’ experience working in social services, particularly with foster youth, at-risk youth, or transitioning youth.

• Experience in case management, youth mentoring, or independent living skills coaching highly desirable.
• Experience working with foster families or in a foster care system setting.

• Strong knowledge of foster care systems, resources, and available services for youth transitioning out of care.
• Ability to engage and build rapport with youth and foster families from diverse backgrounds.
• Strong attention to detail with a focus on accuracy and integrity
• Excellent communication skills, both written and verbal, for training, advocacy, and collaborating with teams.
• Organizational skills with the ability to manage multiple cases and priorities simultaneously.
• Knowledge of trauma-informed care principles and the ability to apply them in practice.
• Strong problem-solving abilities, with a proactive approach to addressing challenges faced by youth.
• Ability to work independently and as part of a multidisciplinary team.
• Comfortable with delivering trainings and presentations to youth and families.

• Environment: Hybrid: Home-based office, Community Settings 
• Range of Schedule:  Mon – Fri, 7:00 am to 8:00 pm
• Travel: Frequent - Uses own vehicle on agency business but does not transport clients.  (Must have a valid Texas driver license and current auto insurance)
