Job Description - Travel LPN - Case Management

The Travel LPN – Case Management is responsible for supporting care coordination activities, assisting with patient assessments, and facilitating effective transitions of care across healthcare settings. This role works collaboratively with RNs, case managers, physicians, and interdisciplinary teams to ensure quality patient outcomes while maintaining compliance with regulatory and payer requirements. The position requires flexibility to travel to assigned facilities for contract-based assignments.

Essential Duties and Responsibilities:

Assist with patient assessments, care planning, and coordination of services.

Monitor patient progress and report changes in condition to the RN or Case Manager.

Facilitate discharge planning and follow-up care coordination.

Communicate with patients, families, and community resources to ensure continuity of care.

Support utilization review processes and documentation requirements.

Maintain accurate, timely documentation in the electronic medical record (EMR).

Ensure compliance with CMS, Medicare/Medicaid, and facility policies.

Participate in interdisciplinary team meetings.

Adhere to infection control and patient safety standards.

Qualifications:

Active LPN/LVN license in the state of assignment (compact license preferred).

Minimum of 1–2 years of clinical nursing experience.

Prior experience in case management, care coordination, home health, managed care, or discharge planning preferred.

Current BLS certification.

Knowledge of Medicare, Medicaid, and insurance authorization processes preferred.

Strong documentation and computer skills.
