Job Description - Resident Care Coordinator

Summary: The Resident Care Coordinator (RCC) facilitates the personal well-being and attends to the non-medical needs of the Assisted Living resident in accordance with the governing federal, state, local, and facility standards, guidelines and regulations.

Essential Job Functions: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Responsible for complete case management services including, but not limited to, pre-move-in and move-in services, providing input for ongoing assessments, move-out or transfer planning, and special and/or urgent meetings.

2. Provides ongoing chart maintenance and audits for compliance with all applicable rules and regulations. Charts pertinent information on residents.

3. Provides marketing information to all Assisted Living inquiries as needed.

4. Ensures the availability of move-in packets, assessment packets, and resident handbooks.

5. Assures new residents receive a welcome and an introduction to staff and other residents.

6. Explains evacuation, disaster, and fire drill procedures to new residents and/or their responsible party and revisits the information as necessary. Participates in drills and drill evaluations.

7. Initiates and updates the resident's profile and face sheets regarding changes in demographics or status.

8. Prepares initial and ongoing Service Plans for residents in coordination with the Assisted Living/Memory Care Administrator and/or applicable staff.

9. Sends notification of Service Plan meetings to family members and coordinates monthly Service Plan meetings.

10. Conducts Level of Care assessments as resident needs change in coordination with the Assisted Living/Memory Care Administrator, RN, and/or applicable staff.

11. Serves as the main liaison for Assisted Living operations and residents, family members, staff, and volunteers.

12. Assists residents with transportation arrangements as needed.

13. Reports weekly census. 14. Prepares monthly financial reporting, which includes funding sources, balance forwarded, a list of Medicaid, private pay, guest meal, medical supply, and admission fees/charges as applicable, and any necessary adjustments to the Business Office.

15. Assists with special activities for residents and community involvement.

16. Supports staff with personal care duties as needed.

17. Participates in staff training.

18. Assists with Assisted Living and Memory Care Center Service Plan meetings as needed.

19. Assists in Abuse Investigation process.

20. Supports & assists the administrator to address and process complaints.

21. Assists administrator as requested on special projects.

22. Works collaboratively with the Resident Services Coordinator for Assisted Living and the Resident Services Coordinator for Memory Care.

23. Other duties and projects as assigned.

Working Conditions:

1. May be exposed to infectious waste and disease (including HBV and HIV).

2. Moves throughout the Assisted Living facility during the work day.

3. Must be able to cope with emotional and/or hostile residents, families, staff, guests, and other visitors.

4. Will be required to work after regular work hours, on holidays, and weekends as necessary.

5. Must be able to lift up to 40-50 pounds in a team effort.

6. Stands, bends, stoops, twists, and turns constantly throughout the day in performing regular duties.

Qualifications

Educational and Experience Requirements:

1. Minimum of 1 years of experience in senior care.

2. Two year degree in Social Services or related field preferred.

3. Experience serving seniors with Alzheimer's and/or dementia.

Other Requirements:

1. Must possess the ability to express kindness, respect, and tact with residents, families, and staff.

2. Must have excellent written and oral communication ability. 3. Must have leadership ability and a professional demeanor.

4. Must possess strong moral and ethical character in executing job responsibilities.

5. Must possess ability to make sound and consistent decisions.

6. Must have an aptitude to adopt and promote community-based care philosophies and best practices.
