Resident Care Coordinator
Type: Full-Time Shift/Schedule: Tuesday-Saturday or Sunday-Thursday, Availability for weekend/on-call/call-outs coverage required Location:Avamere at Park Place - 8445 SW Hemlock St., Portland, OR 97223 - Near Tigard, Oregon
Document information pertaining to changes in residents’ status, needs and appointments
Ensure proper documentation of all care plans, nurses notes and all other resident correspondence
Strong experienced as a Medication Aide
Provide orientation and on the job training to Caregivers and Medication Aides
Assist with daily living activities (i.e., bathing, dressing, and eating)
Measure, record and assist with wellness clinics
Conduct weekly, monthly, and quarterly audits
Manage scheduling of clinical staff
Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy
Maintain confidentiality of all resident care in accordance with HIPAA guidelines
Complete on-going training as assigned
Complete other duties as assigned
Qualifications:
Must be at least 18 years of age
High-School diploma or equivalent
HCA or CNA license preferred
Must have at least 1 year of Lead Med Tech/RCC experience in senior living
Must have or obtain First Aid & CPR Certification within 30 days of employment
Must have or obtain a Food Handlers Card within 30 days of employment
