Key Responsibilities:

• Health Care Planning: Develop, evaluate, and implement health care plans for individual patients.
• Communication: Act as a liaison between families, doctors, providers, and residents/patients.
• Documentation: Ensure medical records are complete and accurately reflect patient care and current medical conditions.
• Team Collaboration: Coordinate with other clinical staff to implement patient plans of care and provide excellent customer service.

Requirements

Education

Degree in Nursing from an accredited nursing school. RN required.

Licenses/Certification  

Current RN license.

Valid CPR certificate.

Experience

Six months experience in a long-term care environment preferred.

Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
