We are seeking a compassionate and dedicated individual to assess patient eligibility for hospice services based on Medicare Guidelines. In collaboration with our Medical Director, you will admit appropriate patients to hospice care, ensuring they receive compassionate nursing according to their personalized care plans. As part of our interdisciplinary team of healthcare professionals, you will play a key role in providing comprehensive and coordinated care. Your responsibilities will include accurate and timely documentation of patient care, adhering to professional and regulatory standards.

SCHEDULE:

The schedule for this position entails Monday through Friday day shifts. On-call duties, which may include after-hours and weekend work, will be rotated based on location. 

COMPENSATION (COLORADO ONLY):

Competitive compensation commensurate with experience and expertise. Our starting range is between $40.87 to $50.48 per hour.

QUALIFICATIONS AND LICENSURE:

• Current RN Licensure with state of employment with no encumbrance
• Previous nursing experience, preferably in hospice or palliative care, highly preferred.
• Compassionate and empathetic nature, with a genuine desire to provide comfort and support to patients and families/loved ones during a challenging time.
• Valid driver’s license with current automobile insurance
• Reliable vehicle
