Job Description - RN Admissions Liaison

Description

**The hours for this position will be Monday through Friday, 9 am to 6pm.**

 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  

The Admission Liaison must be able to perform each essential duty satisfactorily. The requirements listed below are representative of knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

· Interact with hospitals, skilled nursing homes, physicians, and clinics for the purpose of generating appropriate patient referrals/admissions by establishing and maintaining professional relationships with all referral sources.

· Perform community outreach activities including presentations, exhibits and one-on-one contact with community groups, and coordinate special events.

· Provide increased awareness among community partners such as (physicians, case managers, skilled facilities and other community resources) of Hospice Brazos Valley’s scope of services provided to the terminally ill and their family. 

· Coordinate all services provided to patients/families in all settings to ensure quality of care and services as well as appropriateness of care.

· Supervise the implementation of the approved plan of care, modifies as needed, and contacts physician when appropriate.

· Provide supervision of Hospice Aides and Licensed Vocational Nurses.

· Observe for safety hazards in the patient’s and organization’s environments and takes appropriate steps to reduce or eliminate them.

· Attend and participate in in-services, staff meetings, and patient care conferences.

· Complete all required documentation including admission forms, assessments, care plans, and progress notes, ensuring that all records are accurate, timely, and in compliance with hospice regulations and policies.

· Administers prescribed medications and treatments according to law and as permitted within the scope of the individual’s license. 

· Review medical records, communicate with referring physicians, and educate patients and families about hospice services, explaining the scope of care, benefits, and the admission process.

· Initiate plan of care, assess for care needs and changes in physical and emotional condition, provides appropriate nursing follow- up to changes. Instructs caregivers in managing patient needs and submits appropriate documentation in a timely manner.. 

· Educate patients and families on pain management, symptom control, and end-of-life care options. Answer questions and provide emotional support during the admission process.

· Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to maintain comfort. Coordinates changes with IDG to ensure plan of care is meeting the patient’s needs.

· Pronounce time of death following the guidelines outlined in Policy and Procedures.

Requirements

  • Graduate from an accredited school of nursing with a current RN License in the      State of Texas, with no restrictions. BSN preferred. Certification in Hospice and Palliative Care desired. 
  • Two years of nursing in home health or hospice experience preferred.
  • Valid Texas driver’s license, auto liability insurance, safe driving record and reliable transportation required.
  • Knowledge of  Microsoft suite; ability to pull reports and track information
  • Excellent clinical and documentation skills
