What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are a popular option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly daily both at our state-of-the-art day center on Garfield near Hammond during regular business hours Monday through Friday and as needed in their home.

What does an RN Case Manager at PACE North do?

• The RN Case Manager is a care decision maker as part of our interdisciplinary team! What does that mean? Instead of simply following the care instructions, you’re an equal decision-making partner and can help shape the care plan!
• You’ll also collaborate with staff who provide care, evaluate outcomes, and build relationships with both participants and their family members.
• This role conducts initial and periodic assessments and obtains and monitors supplies and equipment. It is also full of inspiring advocacy and education opportunities!
• In this role you’ll be visiting participant homes and participating in our remote on-call rotation.
• You’ll facilitate care transitions, including hospital to home discharge planning that ensures continuity and prevents care gaps.
• Finally, you'll be able to provide PACE North participants with the most cheerful and professional leadership they've ever received!
• Please note this is a full-time, in-person opportunity. It is not remote.

Requirements

What qualifications should you bring to the table?

• You should have a current, valid RN license, and a high school diploma or equivalent.
• We’re looking for:

1. A critical thinker, someone who prioritizes the right answer instead of who gave the answer.
2. Someone who has a higher level of risk tolerance and is comfortable in the grey areas.
